How to avoid the most common RACI mistakes
Three rules that catch 90% of the bad ones. One A per row. If two people are accountable, neither is. R and A can be the same person. Often they are. Resist sprinkling C and I. Every C is a meeting you owe. Every I is an email you owe. Keep the column count small.
